Некоторые авторы в качестве особого вида описывают так называемую пруригинозную экзему, которая часто развивается у детей, больных ихтиозом. Высыпь локализуется на лице (на лбу), с наружной стороны конечностей и местами на туловище.
Высыпные элементы — узелки, покрытые корочкой после расчесов. Зуд весьма сильный, приступами. Позже развивается типичный невродермит; отмечается, что пруригинозная экзема очень близка к невродермиту, так что часто невозможно раз граничить оба заболевания.
Говоря о невродермите, мы отметили пруригинозную экзему в качестве его синонима и считаем, что она является ранним экземоподобным выражением почесухи Бенье.
Интертригинозная экзема (Eczema intertriginosum)
В некоторых руководствах в качестве особого вида описывается и интертригинозная экзема. Приводимые описания говорят о том, что под этим термином подразумевают опрелости, ягодичную эритему, некоторые локализации невродермита и истинной экземы, а также и так называемую микробную экзему.
Стало быть, речь идет об определенном расположении некоторых основных типов экзем у грудных и маленьких детей, а не об отдельном виде экземы.
Желательно, чтобы врач стремился к диагностированию основного вида экземной реакции, не удовлетворяясь лишь диагнозом, определяющим локализацию экземного процесса.
Золотушная экзема (Eczema scrofulosorum)
Так более старые авторы называют микробную экзему, развивающуюся у детей с туберкулезной интоксикацией, со сниженной резистентностью к инфекции и склонностью к эксудативной, экземной реакции кожи.
Этот тип экземы локализуется обычно в центральной части лица, которая именно остается непораженной при истинной экземе. Губы, в особенности верхняя, припухают, выворачиваются, краснеют, появляются трещины на красной кайме и в области носогубных складок; наблюдаются мокнутие и наличие пустулок и корок. Слизистая ноздрей также покрасневшая, припухшая, мокнущая, покрытая корками. Отмечается обычно conjunctivitis phlyctaenulosa, опухание лимфатических узлов, хронический ринит.
Эта экземная реакция часто с трудом поддается лечению и улучшается главным образом с укреплением общего состояния ребенка. Местная терапия — антисептическая.
